The Patient and Family Advisory Council (PFAC) consists of patients and family members who have had recent experiences with Harris Health System. PFAC members represent the voice of patients and can provide input on programs, policies, procedures and processes that contribute to a positive patient experience. Examples of PFAC agenda items: Discharge planning folder Inpatient whiteboards E-quality boards Messaging to patients Meal planning and selection Nurses and doctors listening New patient hub Patient education Who should join? Harris Health patients or family members who have had a recent experience in our hospitals and can: Share their experience in a constructive way Participate in a monthly meeting with hospital leadership Educate staff by providing honest feedback about experiences Listen respectfully to other perspectives Engage in hospital projects and committees as needed If you feel as if you meet these qualifications, please respond and we can discuss ways to proceed. ACS Virtual Volunteer DEPARTMENT: ACS LOCATION: Virtual/ Hybrid SUPERVISOR: Volunteer Champion at Specified Clinic HOURS: Flexible, 2- 3 hour shifts; Monday - Friday / 8am - 5pm GENERAL SUMMARY: Volunteers will support clinics and staff by helping reduce the No-Show Rate for clinics by contacting patients via telephone for appointment reminders, enrollment in MyHealth, reminders for appointments, follow up surveys, etc. Service could be completed virtually or in-person based on the needs and abilities of the clinic. PRINCIPAL DUTIES AND RESPONSIBILITIES: Communicate by telephone with patients of Harris Health System to remind of important clinic appointments and/or follow up calls. May also include assistance with enrollment into our electronic medical records system. May also include providing reminders such bringing a sample to an appointment or taking medication prior to appointment. Document information provided by patients, and keep records of phone call attempts. Ask required questions and follow guidelines regarding patient-friendly language. Use language interpretation call center as needed to communicate in a patient’s preferred language. Attend trainings to include but not limited to orientation, using remote access tools, and department-specific requirements Engage in service activities as identified by the needs of the organization Assist with other projects as requested QUALIFICATIONS: 18 years of age or older Owns or has full access to a telephone, computer, and internet in good working order. Excellent computer and telephone skills Must have ability to communicate effectively and clearly over the telephone Must be able to follow up by phone or email with staff and leadership to effectively communicate any patient concerns or issues Bilingual a plus Good organizational skills, enjoys assisting with patients/visitors questions. Ability to work independently TRAINING: Volunteer Services will provide: Online Volunteer Orientation and Virtual Program Volunteer Training Assigned Supervisor Department-specific training prior to or on start date (phone scripts, computer/program training, etc.) On the job training Prepared and assigned tasks

Ben Taub Tower Volunteer DEPARTMENT: Ben Taub Tower Clinic Administration LOCATION: BT Specialty Clinics; Ben Taub Tower HOURS: Monday and Tuesday 9am-1pm, Wednesday and Thursday 9am-2:30pm GENERAL SUMMARY: Volunteers assist Clinic Administration, Patient Care Technicians and nurses with daily tasks. Volunteers provide assistance and wayfinding for patients who go down to radiology in Ben Taub Hospital, provide snacks to patients in waiting rooms, provide updates to patients, etc. PRINCIPAL DUTIES AND RESPONSIBILITIES: * Providing navigation and wayfinding assistance to patients, particularly to Radiology Services * Speaking with patient families and answering questions * Answering the phone and providing reception and filing as needed * Assisting PCTs and nurses as needed * Follows all instructions and policies QUALIFICATIONS: * Ability to work unsupervised for a short time * Mature, outgoing and dependable, with good interpersonal skills * Ability to communicate verbally with staff, physicians, and visitors in a courteous manner * Dependable, conscientious and diligent in the performance of duties TRAINING: Volunteer Services will provide: * New Volunteer Orientation * Criminal Background Screening * Volunteer ID Badge * Interview with assignment screening and scheduling Occupational Health Services will provide Health Screening: * TB Titer * Proof of Immunization (MMR, Varicella, and Pertussis) Assigned department will provide: * Tour of the department * Introduction to staff members * Specifics on safety * On the job training * Prepared and assigned tasks

Administrative Volunteer - ACS DEPARTMENT: Volunteer Services LOCATION: Ambulatory Care Services Health Center SUPERVISOR: Assigned to Volunteer Champions for projects as needed HOURS: During normal clinic hours: 8:00am-5:00pm GENERAL SUMMARY: The Administrative Volunteer provides daily operations assistance to clinical departments that are non-patient care depending on organizational project deadlines. Typical placements may include: front desk laboratory or where needed. In addition, they may also assist patients with management of their heatlh care through the Harris Health MyHealth portal, our online server that allows patients to view their health records and lab results, schedule appointments, etc. PRINCIPAL DUTIES AND RESPONSIBILITIES: Provide administrative/clerical assistance to assigned work within the department Duties include filing, data entry, copying, shredding of documents and other clerical duties Use of office equipment such as computer,copier, fax scanner, and phone systems Assignment may include greeting and patient navigation of the facility Assignment may include making reminder call to patients regarding forthecoming appointments Assignment may include assisting with MyHealth registration Ability to work one-on-one with patients to facilitate and set up MyHealth account Answer questions pertinent to MyHealth Check out and return Laptop for MyHealth Maintain work area in a neat, clean and orderly manner. Follow Harris Health System policies regarding personal visitors, phone calls, and food/drink. QUALIFICATIONS: Mature, outgoing and dependable, with good interpersonal skills Ability to communicate verbally with staff and physicians in a courteous manner Dependable, conscientious and diligent in the performance of duties Good organizational skills, enjoys working with the public and assisting people Ability to work unsupervised for short periods of time. Successful completion of the Harris Health System volunteer application & screening process and volunteer general orientation. PHYSICAL DEMANDS AND WORKING CONDITIONS Emotional Demands: Demonstrate emotional stability by being able to function appropriately and effectively in dealing with critical, stressful or unexpected situations and with due consideration for patients, visitors, and personnel in vicinity. Requires lifting of up to 10 lbs., occasionally lifting and carrying 10-20 lbs. Ability to stand/walk for at least 10 minutes at a time. Patient Navigator/ Bilingual Patient Navigator DEPARTMENT: Volunteer Services LOCATION: Ambulatory Care Service Health Centers VOLUNTEER CHAMPION: Designated Administrator at Each Clinic HOURS: Scheduling Completed through Volunteer Resources GENERAL SUMMARY: Patient navigators help to improve our patient/visitor experience by helping patients navigate appointments and visits throughout the facility. In addition, they may also assist patients with management of their heatlh care through the Harris Health MyHealth portal, our online server which allows patients to view their health records, lab results, schedule appointments, etc. Patient Navigators are needed in many languages, English, Spanish, etc. PRINCIPAL DUTIES AND RESPONSIBILITIES: Greet Incoming patients/vistors Give out information and answer inquries Assist clinic administration with special projects Escort patients to proper departments throught the facility Check out wheel chairs Work one-on-one with patients to facilitate and set up MyHealth account Answer questions pertinent to MyHealth Check out and return Lap top for MyHealth Maintain work area in a neat, clean and orderly manner Follow Harris Health System policies regarding personal visitors, phone calls, and food/drink .
