The Case Manager's position functions as a collaborative team member with Serenity House women and children's home staff. We are seeking to find a person with experience. Our program provides transitional housing for women that are displaced or facing houselessness. Our small home is a safe haven for three women and three children. These women are working towards their personal goals in a timely manner to achieve self-sufficiency through being employed and acquiring a residence of their own. We are seeking to find a female (she/her) experienced Case Manager to join our team that demonstrates experience initiating and sustaining a high level of accountability resulting in positive outcomes. The Case Manager would coordinate follow up steps with the three women individually to meet the program requirements. Maintaining confidentiality of the clients and records is a must. For the security of the home and the clients the house is in San Dimas however the address isn't listed. Once an interview is scheduled, we will release the address.

Description

Serenity House Transitional Living Home for women and children is our pride and joy. This program provides a safe living environment for women 18-35 years old that are displaced or facing homelessness. Our 6 to 18 month program provides a holistic approach to equip women with life skills, tools and strategies to help them develop a roadmap to thrive on their own once they exit our program.
