Description
The Project Connect model provides a framework that ensures continuity of care for youth transitioning from residential serv ices to their home community. The Project Connect Transition Specialist will assist the youth in developing the Community Action Team (CAT) of volunteers who will support the youth's transition back to the community. The Transition Specialist will also assist the youth in developing his/her Individualized Service Plan which outlines specific goals relating to Project Connect's main four components: Education, Vocation/Employment, Mentoring, and Transportation. Project Connect provides direct services to the youth including GED preparation and Food Handlers certification. Project Connect also promotes employability, life skills, and independent living skills training through the Casey Life Skills curriculum. Project Connect also facilitates important linkages for the youth to explore career and mentoring services through the local One Stop Career Center and pro-social activities.
