Mission Statement
The mission of Colorado Health Network (CHN) is to equitably meet the evolving needs of people affected by HIV and other health conditions through prevention, care and advocacy.
Description
Colorado Health Network (CHN) is a statewide organization in Colorado, serving nearly 4,000 individuals living with HIV/AIDS, and those at risk, as well as other program specific populations. CHN provides innovative, individualized services to those most in need, educates high risk populations, and advocates for social and health care equity.
CHN empowers those living with or at risk of HIV to live healthier, enhanced lives by providing supportive services to those living with HIV/AIDS, and HIV and other health-related prevention and education services.
CHN supports nearly 4,000 clients living with HIV each year, with oral health care services and through support services such as as case management, mental health and substance abuse counseling, housing assistance, transportation, nutrition services, and financial assistance. Each support service includes the ultimate goals of working with clients to make healthy choices, increase their stability, and access healthcare for enhanced, healthier lives. Since 1983, CHN has served more than 75 percent of the men, women, and children who have lived with HIV/AIDS in Colorado.
CHN helps people live healthier lives is by educating those at risk of HIV, and working with them to choose safer behaviors so that they can decrease that risk over time. CHN also provides regular HIV testing and STI testing to ensure as many people as possible are aware of their HIV status.
