Description
Kaleida Health operates two long-term care facilities: HighPointe on Michigan in Buffalo and DeGraff Skilled Nursing Facility in North Tonawanda.
HighPointe on Michigan is a 300-bed facility located directly next to Buffalo General Hospital and the Buffalo Niagara Medical Campus. HighPointe provides varying levels of care for both the adult and pediatric population, including ventilator-dependent or medically complex care, subacute care, dementia care, and traditional long-term care.
DeGraff Rehabilitation & Skilled Nursing Facility is an 80-bed facility, with one floor dedicated to subacute rehabilitation and one to long-term residents. Residents receive expert care and personal attention in comfortable, home-like surroundings. In both facilities (colloquially referred to as nursing homes), individual plans of care are developed by a professional team to meet the physical, emotional, social and spiritual needs of each resident.
