The hospice model involves a team-based approach to expert medical care, pain management, and emotional and spiritual care expressly tailored to the patient's needs and wishes, while also supporting the patient's family and caregivers through the end-of-life process.
Description
Bristol Hospice programs provide hospice services to meet the physical, psychosocial, and spiritual needs of our patients and their families and caregivers. An interdisciplinary group of professionals and volunteers develops an individualized plan of care which includes, as appropriate, the following services:

> Counseling and Supportive Services
> Spiritual Counseling
> Therapy Services
> Personal Care/Home Health Aide Services
> Volunteer Services
> Supplies, Medication and Durable Medical
