Crossroads Hospice and Palliative Care of Philadelphia is a proud to work in partnership with We Honor Vets; Crossroads seeks to match Veterans of the Armed Forces with current and former service members. Veteran volunteers have the unique skills and ability to relate and connect with the veterans we care for, allowing for a compassionate and positive relationship to grow and support individuals in their time of need.
No one deserves to feel alone in their time of need, especially not those brave individuals who have served our country. Read a book, sing a song, play a game, or simply listen. Every minute of time you give means more than you could imagine.
Volunteers are not responsible for the medical care of patients; this includes medication, and personal hygiene. Crossroads Hospice and Palliative Care Volunteers provide companionship, socialization, celebration of life, and emotional support.

When Crossroads began its hospice program in 1995, it was founded with two guiding principles: 1. You shouldn’t die in pain. 2. You shouldn’t die alone.
Description
Crossroads has created a unique staffing model that prioritizes time with the patient. Statistics show that Crossroads ranks among the nation’s leaders in daily visits, providing 16% more care than the national average for hospice providers. In the patient’s last seven days, our visits increase, providing two-and-a-half times more care than the national average. In fact, when we recognize the symptoms that a patient’s death is drawing near, we activate our Evenmore Care program, which allows us to keep a member of the Crossroads team bedside to support the patient and their family through this final transition. In addition to providing patients with physical, emotional and spiritual support, Crossroads offers unique programs like the Gift of a Day, Life Journals, and the Gift of Courage to enrich the patient’s final days by celebrating their memories and making new ones.
