Our AmeriCorps Health Fellows program is a community service and professional development opportunity wrapped into one. This experience enables individuals to connect our patients and communities in need to services to increase access to healthcare services, and health benefits programs, and promote prevention and wellness; while developing skills and exposure to the healthcare field.

We are recruiting for 20 full-time in-person positions (40 hours/week, f lexibility may be available based upon position) and require a one-year commitment to serve a minimum of 900 service hours over an 6-month period. We are looking for individuals who are passionate about health equity and social justice and are considering public health, medicine, social work, and mental/behavioral health for their higher education/career goals. The duration of this program is January2024 to June 2024. This is not an unpaid volunteer opportunity.

Responsibilities

  • Health Coaching - Provide health coaching in one-on-one or group settings.
  • Care Coordination - Provide health screening and connect patients to health care benefits and services.
  • Case Management/Patient Navigation - Assist individuals with navigating health-related services/programs; provide eligibility assistance and enrollment in health insurance, coverage, and other health benefits programs.
  • Community Engagement & Outreach - Conduct outreach in the community (i.e. schools, community centers, events, and shelters); provide education about available health resources and related programs and health care options.
  • COVID-19 Response - Support COVID-19-related activities such as vaccination efforts, providing educational information, and linking patients to health services and/or social resources (i.e. food, transportation, etc.)
  • Volunteer Recruitment/Management - Assist with recruitment, training, and retention of non-AmeriCorps volunteers; assist with the management of non-AmeriCorps volunteers.
  • Service Projects - Participate in various service projects; coordinate and engage fellows in planning and participating in community service projects.
  • Training - Attend trainings on various health topics and resources as needed for placement, plus professional development training as available.

Qualifications

  • Commit to 900 hours of service; work full-time (40 hours/week, flexibility may be available based upon position); commit to a 6-month term of service.
  • Must be 18 years of age, or at least 17 years old with the signature of a legal guardian.
  • Must have a high school diploma, GED or commitment to obtain during their service year.
  • U.S. citizen, U.S. national, or lawful permanent resident.
  • Successful completion of background check (upon hire).

In accordance with LifeLong Medical Care's commitment to provide and maintain a workplace that is free of known hazards, we have adopted a Mandatory COVID-19 Vaccine Policy to safeguard the health of our employees and their families; our patients and visitors; and the community at large from infectious diseases, that vaccinations may reduce. This policy will comply with all applicable laws and is based on guidance from the Centers for Disease Control and Order of the California State Public Health Officer. Unless reasonable medical or religious accommodation is approved, all employees must receive COVID-19 vaccinations.
