Chapters Health Hospice has expanded to Pinellas County! If you are interested in honoring Veteran's and First Responders, this is the place for you! Our Valor Program utilizes volunteers who are veterans and first responders to provide peer to peer visits to our patients. Volunteers also assist with pinning ceremonies to honor the service and sacrifices made by veterans and first responders, at which time the person of honor receives a pin and certification of appreciation. The Valor Program provides for community outreach and awareness to help understand the unique needs of vets and first responders. If you would like more information, please contact Luanne Lane at lanel@chaptershealth.org .

Mission Statement
HPH Hospice is where excellence in compassionate care maximizes quality of life
Description
HPH Hospice, founded in 1982, is a not-for-profit organization offering medical, emotional, social and spiritual support services to patients and families living in Pasco, Hernando and Citrus counties. HPH Hospice cares for families and patients affected by life-limiting illness, grief or loss. We care for the majority of our patients in their own home, surrounded by the things and the people they love. We also provide care to patients in nursing homes and assisted living facilities and in our Hospice Care Centers.
We believe dying is a uniquely significant time in every person’s life. We also believe caring for the dying requires a different pace and set of skills than other kinds of health care, with primary emphasis on physical comfort and emotional support. We believe that every individual deserves to receive care without feeling demeaned or like a burden. That family and friends need caring support, tooCAUSE AREAS
