Join our amazing team! Do you want to make a positive change in the lives of older adults and their care partners? The Geriatric Assessment Program (GAP) is a free, comprehensive medical assessment program for older adults, 60 years of age and older, and their caregivers (if applicable). GAP is funded through a grant from the Nevada Aging & Disability Services Division. This 10+ year program is an extremely valuable program for older adults who have complicated medical issues/conditions, multiple medications, recent/multiple falls, hospitalizations, physical/cognitive/emotional changes. The Geriatric Nurse coordinates with the patient and/or caregiver, GAP Social Worker, GAP Occupational Therapist, GAP Geriatric Physician and the GAP Special Projects Manager.
Our GAP Nurse takes patient vitals and reviews patient medical history & medications and collects intake paperwork. *There is some financial compensation currently available for this position.

- This position requires 12 to 20 hours a month. Must be flexible and EXTREMELY RELIABLE. Clinic dates are typically 2 days a month (Thursdays only) with some follow-up clerical required.
- We are looking for an experienced, licensed nurse, who enjoys with working with older adults. Familiarity working with adults who are experiencing cognitive losses is very helpful. This is a very rewarding program with an amazing, dedicated team.
