ORGANIZATION: LifePath Hospice, Inc.

Do you enjoy helping those in need? Do you like talking on the phone? Then LifePath Hospice has the volunteer job for you! LifePath Hospice is seeking volunteers to assist with our various calling programs, such as "Tuck-In", "At Home with CHF", and bereavement calling programs.

Volunteers "Tuck" in hospice patients each Wednesday by assessing their need for medication, supplies, equipment to ensure they have what they need through the weekend. Our "At Home with CHF" callers make daily calls to our CHF patients to check in with them, assess and document any changes in condition, and to help educate patients on the program goals to help manage the condition and reduce visits to the emergency room. Bereavement support volunteers offer condolences with compassion and provide information on LifePath’s individual grief counseling, support groups, and children’s grief programming.

Volunteer callers are a vital resource to the hospice team and help us provide additional layer of comfort and support to patients and families

Description

LifePath Hospice offers guidance and support to individuals with life-limiting illnesses and their families as they travel on a new and unfamiliar path of life. With the assistance of medical professionals, counselors, clergy and trained volunteers, patients and families are cared for and educated on end-of-life issues in order to make this final journey as peaceful and comfortable as possible.
