The Take Charge of Your Health Programs are evidence-based workshops given two and a half hours, once a week, for six weeks. Workshops take place in community settings like senior centers, churches, libraries and hospitals throughout Cook County. Workshops are presented by two trained facilitators, one or both of whom are non-health professionals with ongoing health conditions themselves . The workshops give participants the skills to coordinate all the things they need to manage their health, as well as to help them keep active in their lives.
Volunteer facilitators will be expected to be trained in the Take Charge of Your Health Series workshops (Chronic Conditions, Cancer, Diabetes and Chronic Pain), and will be expected to commit to facilitating at least 2 workshops per year ; workshops are facilitated in English and Spanish . (Note: W orkshops are facilitated virtually via zoom and in-person throughout suburban C ook C ounty . )

Description
AgeOptions (www.ageoptions.org) is a not-for-profit organization located in Oak Park, Illinois, that has served the older adults of suburban Cook County and their families as an Area Agency on Aging since 1974. As part of the nationwide network, we are authorized by the federal Older Americans Act and the Illinois Department on Aging as the Planning and Service Area for the 30 townships surrounding Chicago.
AgeOptions is governed by a Board of Directors, whose responsibilities include setting our direction, establishing policies, and managing and allocating resources. Using proven methods of research and planning, AgeOptions is entrusted with distributing funds to a network of organizations that serve older adults and their families. Our Advisory Council consists of a group of older adults who represent and link us to the communities in our service area. Our executive and management team, whose combined experience in the field of aging exceeds 120 years, are responsible for the implementation of strategic plans and day-to-day operations.

- Orientation or Training
- Complete the Take Charge facilitator trainings based on interest. In person leader training meets for 32 hours over 4 days, virtual leader training is 32.5 hours over 6 weeks. Facilitate at least 2 full workshops within a year of training.
- Be on time, dependable, and report to assigned workshops as scheduled. Abide by HIPAA guidelines. Motivated and has an interest in serving older adults. Comfortable with public speaking, able to read, follow and deliver material from a manual.
